About the Role:

  • You will provide individualised care and education for people living with type 1, type 2, and gestational diabetes in both outpatient and inpatient settings.
  • This important role aims to improve outcomes for people with diabetes and other chronic diseases through the provision of assessment referral and clinical services.
  • This role also educates and empowers health professionals, carers, and the general community to better manage diabetes.
  • You will be supported clinically by the specialised diabetes team including credentialled diabetes educator and nurse practitioner, as well as the range of senior nursing staff of various specialties at community health.
  • Operationally, you will be supported by the Armidale Community Health nurse manager who will identify and support educational opportunities as they arise.

Where you’ll be working:

  • You will join the specialised diabetes service at Armidale Community Health. The local diabetes educator covers Armidale, Guyra, Uralla, working closely with local dietetics services and the Tablelands Diabetes Nurse Practitioner.
  • Armidale is the main hub of the New England High Country, and is one of Australia’s most elegant regional locations, with its 19th-century churches and heritage architecture.
  • A graceful city of tree-lined streets, Armidale is a five-and-a-half hour drive from Sydney and less than 90 minutes from Tamworth, with flights between Sydney and Armidale operating daily.
  • Armidale is surrounded by beautiful forests, mountain gorges, national parks, and waterfalls, including Wollomombi Falls, one of the highest waterfalls in Australia.

About you:

  • Postgraduate qualifications in diabetes management or evidence of working toward same.
  • Demonstrated ability to explain complex diabetes concepts and deliver diabetes education to consumers and other health professionals.
  • Computer literacy skills and knowledge of patient information systems (iPM, chime, DMR, CGM programs)
