Where you’ll be working:

  • This position is based at Tamworth/Nundle Community Health Service as part of the Chronic and Complex Care team. It is a single clinician service however has onsite support from the greater chronic and complex care team of registered and enrolled nurses. There is established links between this role and other respiratory staff and services located across HNELHD.

About the role:

  • This position provides respiratory assessment, care and treatment of patients in Tamworth Hospital and Tamworth community and surrounds.
  • You will manage the outpatient Pulmonary Rehab program and support other outpatient respiratory clinics.
  • A small component of work exists assisting with indigenous chronic disease clinics (3 x year) in Mehi Sector (Narrabri and Moree).
  • You will manage TB services for the community in consultation with Population Health.
  • The role provides clinical leadership, education and expert clinical advice to staff in the Peel and Mehi Sectors as required.
  • You will have access to clinical experts at John Hunter Hospital and Armidale Hospital, online training and education, and Respiratory Stream.

About you:

The successful candidate will possess the following skills/attributes:

  • Clinically skilled in patient respiratory assessment and care delivery.
  • The ability to work with patients across the continuum of care, inpatients through to outpatients.
  • Experience working in the community setting.
  • Patient education skills.
  • The ability to work autonomously.
  • Ability to prioritise workloads.

Requirements:

  • Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
